If you have an immediate need like a refill or a condition for which you need immediate care, please fill out a request on this page, which we will check daily and we will get back to you by email or phone. For emergencies dial 911.

If you are already enrolled as a patient you can use the Patient Hub or Atlas Phone App to directly text your doctor at Oakhaven. If for some reason that is not working for you, you can still use this form as a backup method.

If you would like a consultation before enrolling, with either Dr. Staci Jordan or Dr. Charles Sullivan, please fill out the form below and we will work with you to schedule a time for you to have that conversation. Note that no treatments or diagnoses are available to individuals who are not yet enrolled.
