With Oakhaven Health, your monthly membership covers the care most families rely on year-round—no surprise bills, no rushed visits, and no fighting with insurance. You get unlimited appointments, direct access to your medical doctor, and a level of personal attention that simply isn’t possible in the traditional system. Everyone must be properly enrolled with a form of payment on record, but we do offer some cash pay options, but we never submit billing to insurance. We do not accept Medicaid and do not bill Medicaid. We are opted out of Medicare and do not bill Medicare. As a Medicare opted-out medical practice, our services are not generally reimbursable by Medicare.
These are our pricetags for non-member self pay, which illustrate the high value in becoming a member. There are also some prices that do apply to members, above and beyond the membership fees. For items greater than $249 in price for non-members, a $249 discount applies for members. Members enjoy all items priced at $249 or less at no additional cost, provided their membership is in good standing. To be clear, items priced on the price tags page for less than $250 are free for members in good standing.

Your membership price is a prepayment for the period you select, which can be monthly, quarterly, or annually. Prepayment reserves a spot for you in the patient panel at Oakhaven Health at the agreed prepaid price.
We reserve the right to change our prices, which will impact any future periods that are not yet prepaid.
Proper identification required at your initial visit for newly enrolled patients.
You may cancel your subscription at any time without being charged the remainder of that month's membership fee. At that time access to the patient hub and phone app will be deactivated until you resubscribe. Ability to schedule appointments or directly communicate with our staff will be suspended other than for account communications. Prescription refills may be revoked at the discretion of the prescribing physician.
After a cancellation there will be a reinstatement fee of $150 to reactivate your subscription, if there is availability in our patient panel, which has limited space.
If you have to cancel due to financial hardship, we will waive the reinstatement fee for the first occurrence.
