Enrollment is a three-step process,

Read the patient agreement
If you agree, digitally sign the form below
Continue enrolling in our medical records system (Atlas.MD)

STEP 1

If you are on a mobile phone when you click the "Open Patient Agreement Document" link, your screen will be filled with the printable version of the agreement. Once you finish reading it, press your back button or left arrow on your phone to return to this page and continue to Step 2 below.

If you are on a desktop browser a new tab will be opened. After reading the agreement, return to this tab to continue with Step 2.

STEP 2

You do not need to login to Google to sign the form even though Google will invite you to do so.

When you complete Step 2 you will be presented with a link to the medical records system enrollment where you will complete Step 3.
